Single Address Field Form

This demo form includes the address field, locator button, Google map field, and the coordinates field. The form will also try and detect the visitor’s location when the page first loads.

The form also includes at the bottom a list of text fields that will be dynamically populated with different location fields using the Dynamic Location Field option of the plugin.
